This form must be filled by Choreographers/Coordinators to register the group performance only. For event entry active members just need to show their digital membership cards to attend the event. Non-members who are part of performance group need to pay the performance fee in this form.

There is no need to collect Member IDs for active members! Just Enter First and Last Name for each performer once you select number tickets in second section (Members + Non Members).

In First Section - Enter all the information related Group (Name, Description, Choregrapher & Coordinator Names etc)

You can take one time memberhsip for Diwali only event before filling up this Form

While Feeling up the entry form, Add following information:

1. Nick Name - Performance Group Name

2. Details : Performance Description 

3. Position: Choreographer, Coordinator

Group Photos & Audio/Media Should be submitted before Oct 1st, 2025.

If you dont submit by the deadline then ICS may not be to include your group photo on the Calender and your performance slides (with performer names) will not be shown at the event during performance.
